Start with the current USCIS instructions
Read the USCIS Form I-693 page and its current instructions before scheduling or submitting the examination. USCIS may update validity, submission, or form-edition guidance, so an older explanation may no longer match the filing requirements.
Use a civil surgeon designated through the USCIS process when the instructions require one. Ask the civil surgeon what records and vaccination information to bring to the appointment.
Bring the records the civil surgeon requests
Organize identity information, vaccination records, prior medical records, and any other material the civil surgeon tells you to bring. The civil surgeon, not a software checklist, determines what is needed for the examination and completes the medical form.
Do not guess about a medical condition, vaccination record, or examination result. Ask the civil surgeon about medical questions and use the current USCIS instructions for questions about submitting Form I-693 with the adjustment packet.
Keep medical material private and organized
Keep the sealed medical packet and any copies according to the civil surgeon's and USCIS's instructions. Do not open a sealed envelope if the instructions say it must remain sealed. Record questions for the civil surgeon or an immigration professional instead of guessing.

- Confirm how the civil surgeon will return the completed form.
- Compare submission instructions with the current I-485 instructions.
Check the applicant-specific packet context
A principal asylee and a derivative applicant may have different I-485 packets and different evidence questions even when they attend medical appointments together. Review the current I-485 instructions for each applicant and ask the civil surgeon how the completed form will be returned for that person.
